How to Access Our Services
Anyone with non-corrective vision loss can access our services by following these steps:
1. Referrals
- People with vision impairment or their families are welcome to contact us directly.
- Optometrists, opticians or GPs can make a referral by completing a Referral for Low Vision Assessment (doc, 189kb).
- Ophthalmologists can make a referral by completing a Low Vision Medical Certificate (Adult) (pdf, 191kb) or Low Vision Medical Certificate (Child) (pdf, 133kb).
2. We will contact you to talk about how we can provide services to you, and our membership fee.

Services to regional areas are provided from our Centre in Victoria Park and from regional offices around the State. We visit rural areas regularly, including the Kimberley and Esperance regions.
